I went in assuming that of course most people get mobile BPs on QR, but now I’m thinking maybe not?

That would go a long way in understating the issue.
I find on international flights its more often I do not get a BP on the app more than I do. Its very heavily dependant on the entry requirements of the destination country. Also, some airports simply don't accept them which is what, I believe, the case is that you've seen at KTM.
