I've also had it happen when using online travel agents like Expedia or Travelocity, rather than buying direct from the airline. I also have a unique situation where I go by my middle name but the name on international tickets is my legal first name (since it's on my passport) so sometimes they're not happy that the name on the ticket differs from the name on the card and want to see the card.
