There is absolutely NO need for middle names to be included in the ticket. IATA regulations state that they are completely optional.

They generally are required when actually travelling (and need to match the ID being used), and can always be added during the check-in process, but for anyone below 18 even that's not going to be a hard and fast requirement given that no ID is required to be shown.
