I've never seen it anywhere either. The bag attendants said its a normal occurrence when the afternoon QR flight comes in. Bags everywhere from the previous flight on that belt, and still on the conveyor l, that they'd have to stop every 5 or so bags because there was nowhere to put them.
Maybe just a one off or I'm just very unlucky at ORD.
