Did you book a Flight and Hotel booking at once and then add the car hire? If not and you booked the flight and the car hire later then its not a holiday aparently it's just two things and not ATOL protected.
Still doesn't explain why you are being asked to pay twice, but it might explain why you didn't get the BA holiday email.
