Can it be combined with each kids base skymiles? Trying not to end up with the cardholder have a lot of points, and everyone else having small unusable amounts. If that's the case, I may just stick with another card for the flexibility.

Anyone's miles can be used to purchase tickets in someone else's name. However, it's not possible to pool miles from different account towards booking awards on one reservation (miles from each account can only be booked towards awards on separate reservations). It's possible to transfer miles between accounts, but unless it's a very small amount of miles, it's usually not worth the transfer cost.
