-someone at Chase is screwing around
-someone is in your email
-someone is in your password manager
-you reset your password to the same password after the first change

Could any of those be the case? I guess #2 wouldn't be something you'd know about, but the others?
Or your computer is compromised with someone having remote access or a keylogger
