I also suspect BRG frontline associates have to meet very demanding quotas for claims processing, and as an approval takes much more time documenting and building a case-file, poorly supervised as they are it is a huge temptation to just deny a claim on a trumped-up reason, close the file and move on to the next claim. And “I couldn’t locate the comparison rate” is a great excuse to do just that.
