They don't ask first. That would make a mockery of the service setup and they'd be zig-zagging over the cabin. Everyone is asked in order then first choice priority is given to Gold or higher.
But, and there is a but here, I often ask crew if they'd help me secure a particular dish when they hand out the menu. I'll do that nicely such as "Oh heard so many good things about X, would love to try that..." and the crew nearly always respond with "between you and me, I'll do my best to make sure that happens!".
Only issue I ever seem to have is over the cheese board. I've only had it once, recently. Wasn't available on a recent J flight and they weren't loaded at all on an F flight to ORD in March.
