And now that upgrade is no longer showing as available - I assume what happened is that someone booked it during the 60 seconds or so I waited for the transfer to complete.

So now I have 36k miles "stuck" and completely unavailable. Going down the Twitter / call center route for now and will see where I end up.
So my miles were finally refunded after roughly 80 hours (and submitting multiple "feedbacks" through their web comment form). They were refunded in 2 batches: 9,000 immediately, and then the remaining 36k in that ~80 hours later time period - these amounts don't seem to line up with anything (the upgrade was 45k and I only had 40 miles in my account at time of transfer).

To other comments offering reasons behind TK's "holding" of miles - I've been a M&S member for years, and this was not the first time I transferred miles for an immediate upgrade (I had just done it a few days prior to this, successfully). My only thought is that I didn't back out of the upgrade screen and re-verify that the upgrade was available after I was sure my miles had successfully transferred - could have been a strange timing thing where I saw the upgrade available, but by the time I clicked "pay" it was already gone.

The upgrade was also priced a bit strangely - one segment was priced at 45k miles, and the next at 11,250. But combined they were still just 45k (no idea if that's normal).

Regardless - I did eventually get my miles back (but still looking for that upgrade...). If this happens to you make sure you submit feedback ASAP on their site, as that seemed to be the only route for getting this resolved.
