I mean technically for mileage tickets too. It's just miles are a refundable currency, much like a refundable cash fare.

What exactly was the rule that was applied to the purchase of the 2 tickets with the 2 Cfts that resulted in 2 different outcomes? I wouldn't want to have my Cft transferred to a less-than-friend for whom I may buy a ticket in future.
Case 1 you were using a single cert, so it just deducted from that certificate. Case 2 it had to combine the two certificates to create one with enough value, and then deducted from that new certificate. Probably an oversight in the combination process that takes the passenger information to put on the combined certificate instead of the existing certificate information.
