There is no way OP is to blame at all, at least if the blame is for putting Herr Doktor as his title, since it is his title, and LH appears to have asked for his title. In the UK systems often give a list of about 15 possible titles, including Lord, Professor, Sir, etc. If I were a Lord and picked Lord as my title, then somehow my boarding pass read "Flying Lord Crooked" rather than Lord Flying Crooked no part of the blame would rest on me.

Maybe your point was that the situation is unintelligible as described so it's reasonable to think OP may have screwed up somehow and doesn't realise it. If so, sure, that seems plausible.
