I never use cash in the US either. But I always carry some cash in Italy. You don't need to bring it in advance - I just go to the ATM at the airport and withdraw some when we land (mind you, my bank refunds ATM fees).

You almost certainly will not need a prepaid SIM. For that usage 5 gigs will last you. I think they throttle you past 5 gigs, not shut you off, but we didn't run into that and some folks in the group were heavy data users.
When you say buy in advance, this isn't like the HEX in London where I'll have to buy like 90 days in advance to get the lowest rate? Do you mean to buy at the machine n the terminal before boarding the train?

It doesn't seem like the rate is distance based and it might be safer to pay €1.50 for each ride. The 24-hour pass requires 5 trips to break even.

You're right, but 256kbps is very slow...I experienced that while in Singapore. Not fun!
