This seemed to be the case for me on entry @ HND on 8/11

Apparently there's only one kiosk and initially it had trouble scanning my fingerprints and passport before telling me there was an error and to try a different kiosk.

An agent came out and reset it and asked me to try again. This time in addition to the scan errors, my TTP card got stuck in the reader.
The agent came out prepared with a ruler with some double sided tape on then end to fish out the card.

After fishing out the card I was asked to sit in the detention room while they found somewhere to stamp my TTP card.
About a half hour later I was summoned and handed back my card with the stamp on it.

This was the first time I attempted to use it and I have to wonder if it was worth the trouble.
