That's interesting. It's been a while since I booked with Amex, but I didn't pay Amex. They made the booking for me, and subsequently posted the $200 travel credit to my Amex account, but nothing was prepaid for the hotel. I paid the hotel bill upon checkout like any other stay.
It depends on the rate. Some rates are prepaid, some are not.
In my case, Amex rate was about 10% more than the hotel rate.
