It can be an issue when they want to make it an issue. Many many years ago I was renewing my US visa & they misspelled my middle name in the renewed visa. It was obviously a mistake on their end because they had all my documents with the correct spelling. My previous visa had the right spelling, my visa application form had the correct spelling, they had details of my Green Card (which I had voluntarily given up a few years prior) which had the right spelling & of course my passport had the right spelling. But once the visa was issued, I was stuck with it for the next 10 years.

I remember at least a dozen occasions where I was questioned on this either at check-in in India or at immigration in the US. But then again there were even more occasions where no one bothered & I traveled question free.
