If they agree to upgrade, do you find they will update the app to reflect an updated room? I'm a little incredulous about this unless it's due to a Suite Night Award. When I've received a confirmed upgrade by using a SNA, then the upgraded room shows up in the app. But I'd be surprised if they would show an upgraded room just by asking. It would be great though! Is this what you've experienced?

In person check in did have advantage though since you could directly ask the FD about upgrade.
As for occupant of the room, I feel that the more requirements you put on the room, like connecting rooms, extra bedding, and double beds instead of 1 king bed reduce the upgrade chance significantly as the hotel try to fulfil these requirements first.
(Note on connecting rooms, sometimes this might goes for your advantage when the hotel only have a standard room connected to a suite)
Since in person check in has the advantage of asking the FD, then that means advance check in by app is actually associated with a lower chance of upgrade, right? Therefore, it's best not to check in early via the app, and instead wait to you arrive at the front desk to ask for an upgrade. Is that right?
Yes, I feel like having a 3rd person itself is like an additional requirement on the room. So I guess by that logic, 2 occupants 1 king bed is likely to afford the greatest chance of upgrade. That's what I'm thinking from my experience so far, anyway.
