While this may or may not be accurate, I began to suspect that the limited choice had something to do with filling out the question above, which was what name do you want to be on the card. With that in mind, what finally worked for me was to skip both that question (desired name on card) and the question about alternative business names and proceed with the rest of the application. At the very end, the application returns you to any required boxes that you failed to complete. At that point, I was finally able to select "no" for the doing business as question, and then after that filled in the desired name for my card, and was able to successfully complete the application. (Pending as of right now). Just passing along my experience in case it is useful for others.
This is interesting - I'm trying to apply for the same card but instead of giving me the error on the D/B/A question as you had issues with, I can't enter 0 for # of employees. The note below the box literally says "Enter the number of all additional employees, not including yourself. Enter "0" if you're the only employee." However, when I type in 0, it gives me the error "[numberOfEmployeesError.NOT_IN_RANGE]" Looks like a coding error, but I'm not sure whether to apply with 1 in the box or somehow call in?
