There are two ways to apply for a business card.
1) If your business is registered in Hawaii so that Chase can verify it through public records, apply using the business's legal name and actual gross revenue numbers. You will ALSO provide your Social Security Number and personal income. Typically, your personal finances will determine if the application is approved.
2) Otherwise, you can apply using ONLY your own First Name + Last Name as the business name (do not add any words), with SSN and personal income. Do not identify the business. This is how others apply for a business card when they do not actually have a business, and the success rate is high.
