There must have been a miscommunication because there's no such thing as a
Surcharge/Payment Transaction Fee Transportation Tax (Domestic/International)
It's

Airline Fuel and Insurance Surcharge / SQ Distribution Fee / MI Distribution Fee / Payment Transaction Fee
and then

Transportation Tax
Are you sure she wasn't talking about the credit card transaction/currency conversion fee? Probably doesn't apply to you, but might have just been part of her script, unless she specifically mentioned the $21.10.
