This is most definitely happening. I've been on more and more flights lately, mostly RJs, where first is showing 7+ seats available yet the upgrade isn't clearing. Meanwhile, the app keeps trying to sell me an upgrade even though I'm within my upgrade window. I admit I now buy first when I truly want first, but this is annoying nonetheless that even getting an upgrade on an RJ for an EP is difficult. I've resigned myself to never getting an upgrade on popular routes, but RJs used to be near guaranteed upgrades.
