It’s a bus journey and another passenger/cabin baggage security rescreening at T3 (after already having done a security rescreening on flight arrival to T5) if doing a T5 to T3 thing in such situation to grab the AA flights to JFK.
When BA has done this to me, they typically kept me in the same class of service on the AA flights out of T3.
The OP’s sister was said to take BA 181 to EWR, and that left out of T5.
