There are direct trains from Heathrow to Custom House, so you do not need to change trains.
If despite this you choose to leave the Underground/Elizabeth line at Liverpool Street station and re-enter the same station you will be charged two journeys. You'll therefore pay an extra £1.60 taking you from the £13.30 Elizabeth line fare to the £14.90 daily cap.
If you re-entered the National Rail part of the station within 40 minutes the rest of your journey would be treated as a continuation, so you could carry on (for example) to Stratford then DLR to Custom House at no extra cost, though this is quite a convoluted way to save £1.60.
