I assume it's the airline, since it's all about the risk they're willing to take in accepting a connecting itinerary. It varies by airport, of course - and by terminals within that - but the event it's managing isn't really one for the airport management to define.
MCTs exist at airport level and any airline specific rules exist as exceptions from the basic rule. It's entirely feasible that airlines have full control over the exceptions, I don't know.
