You might (or might not) need a hotel registration form for opening a bank account. But for sim registration your passport should be enough.
I think you just need a registration form that you can get at any police station. But the last time I was there, the DMV took the original when they were processing my driver's license and the bank required the original so I was unable to open a bank account.
