While it depends on the nature of any disabilities, and how much assistance needs to be given (and what type), I would suggest the window seats on either side of the plane. Have the elder sit in front of the aid, that way the aid can easily see if the elder needs assistance or tries to get out of their seat, etc. If you choose seats in the middle, you will have to constantly walk around the bulkheads to reach the other aisle.
