Total gross annual income refers to your personal income. Chase explains what to include in this amount.

Annual business revenue refers to the business's income.

Annual business revenue is not business income! Annual business revenue is just total sales.

For example, I own a real estate brokerage. We generate well over $1 million in gross commission income(revenue), but that is not income from the business, this is revenue before expenses. They are asking about revenue to determine how much potential spend you could generate and put on your cards.
