I had a similar experience a few months back. Basically, I couldn't travel anymore due to getting into a car accident but the agent I talked to over the phone didn't cancel it. So the gate agent of the flight I was supposed to be on called me saying that she had to manually cancel the flight because I could either fly the rest of it or get it rebooked for free.
