last month hertz had a second reservation opened under my name then closed it and billed me - looks like someone else rental attached to my backup reservation i coulnd not cancel in time...All half overlapping with an actual one i rented - took a couple weeks to sort out, sent them pics of boarding passes but they issued refunds eventually. So yes it is absolutely possible.
