I think that there have been postings here on FT about people with Avis Preferred accounts cancelling a reservation at the last minute and then being charged, but it's possible that they were able to get the charges reversed thereafter.
Apparently some Avis stations auto-activate Preferred contracts about 1-2 hours prior to ETA, the flight number arrival time may trigger that. I have had to pursue refunding of charges on never driven cars.
