I had a similar issue, took forever to get the card.
If this is first time you have used TD, I would call and ask if they have two pieces of ID for you on file. If not, they won't release the card.
If they do, ask them if they know if the card has been sent. They can usually tell when the card has been shipped out. I would also call the branch that is receiving your card to make sure they havent missed it somewhere.

Yes thank you. I called in a few days ago and they told me that i had to go visit a branch with two pieces of ID. After doing so they issued me a replacement card that was shipped overnight. Within 2 days I got my replacement card, but still didn't hear anything about the original one that was supposed to be sent. So all in all it took about 6 weeks from the day I was approved till the day I received mine.
btw, have you tried using your visa to access a maple leaf lounge yet?
