I wouldn't expect that at all. In fact, I'd expect it to be zero, or nearly so. If you were relying on what you were seeing on the United website, don't. Call.

That's key. In that case, you would get a cash refund after converting the fare to a one-way and repricing, and then deducting the cancelation fee. I'd be surprised if this were much more than $0, though.
By far, your best bet is to change the return date on your existing ticket. UA may waive the fare difference due to the medical situation, but even if they won't, it should be small if there is inventory available in your fare class. If you're on a T fare, look for flights back home with T inventory in Expert Mode.
