Partially used intl ticket — options?

I booked a round trip ticket from US to India in paid economy class on 016 stock, with all segments except the first operated by Lufthansa with LH flight numbers. I’ve already flown the outbound and was supposed to return soon. But I’m now legitimately sick, and my doctor advised not to travel.

As you would expect, the fare difference to change the ticket by a few days or a week is astronomical. So I’m wondering if I can get a refund or an ETC for the residual value. (Once I’m better, I’d book an award ticket using United miles to fly back.)

What should I expect from them?
  • Cash refund of the unused ticket portion (ideal), potentially against a fee
  • ETC of the unused portion (that’s okay too)
  • FFC for a future India to USA ticket (not ideal since I won’t have such a trip anytime soon)
  • something else?

Before refunding, would they reprice the ticket to a one way? If so, that probably eats the value of the ticket.

Would medical documentation help? (I’m worried they’d find it suspicious if I fly on an award ticket a few days later, even though it’s actually legitimate)

My confirmation email says “REFUNDABLE/CXL FEE”
