I've done this link twice; once for my business and personal accounts, then again for my wife's accounts. IIRC, I had to call Chase to do this. I was told to set up a business login, then the rep then linked my personal accounts to the business login. I do remember I was told the personal login would not work for linking.

i can still use the personal logon after linking, I just don't.
I presume two unique usernames are required? That could essentially mean "surrendering" a username utilized since the beginning of the Internet on the personal account.
