As a person who suffers from vertigo - I can attest that different staircases have different angles. It can be a problem.
I generally simply ask someone to help with my carryon, so that i can hold on securely.
Once, leaving a B-737 by stairs in the rain in Alaska, I hesitated at the top because I had both a carry-on bag and a personal item, l but needed to hold the rail. An ramp employee (this was on Alaska) came up the stairs to get the bag. It was at an airport where using stairs wasn't typical; that day the jetway was out of service. If I had known that earlier I could have attached a shoulder strap to one of the bags.
