If you had made photocopies of things you once sent in and had an account with an airline, you might be able to convince them that you have done what is neccessary but somehow it could have been lost in the mail. Re send your copies and then write a nice letter that explains something of how that could have happened.

I actually have a problem similar to this one involving a 2 year old British Airways deal with Jaguar. A friend of mine who never really cared for miles when I convinced him to sign up for this 10k deal where you test drive a car has now decided his account, which should have these 10k in it, is actually useful in his life. The miles are not there. BA said if he could find any copies of the form he would have filled out, they would probably credit the miles. He thinks he has it but needs to check his basement or attic.

Point is, if you have a membership already and you can show you flew, it'll come down to COPIES and well-written rather persuasive letters laced with strange but possible circumstances...

My boss asked me to help her organize her miles. She flies all the time and I found out she never keeps track or cares about this stuff. Her offer to me was this:
You help me sort them out and make sure I get my flight credits and you can have some free tickets! She was envious of my recent free trip over thanksgiving and so she handed me a bunch of boarding passes.

I have my work cut out for me!
