I think in practice they don't care. Whilst there is a rule (quoted above) that the voucher holder must travel, that is for the purpose that you can't for example book your wife and a mate to travel taking advantage of the 2-4-1, it has to be you + 1. If you are ill/late/whatever they won't be bothered at all as ultimately they don't lose out in anyway, you'll still spend the voucher and the associated avios.

If you phone them in advance however, they may be able to split you off the booking and then refund your taxes/charges and maybe the voucher too - got to be worth asking.
