Quick additions to the comments above;
- at least one of the VOA counters always has a credit card machine and the extra fee is minimal; hence no cash needed (and if so, there are a few ATMs before one gets to the immigration counters)
- grab is on both ends of the terminal; the path to the right into the carpark seems more reliable to me; additionally there is a e-grab counter right in front of the terminal, but the one attempt I tried to get one, it failed; Gojek (a large local competitor) is only towards the left upon exit
