Which is all well and good except that there was no "form below" in the message that I could use to notify them.
Hopefully my own "form" will do the trick.

I sent an email about some segments that posted with missing SQD earlier this week, and like you, was confused about the missing “form below.” While I’m glad I ended up finding it, it sucks that it sucks that it only accepts one segment at a time, and you have to re-enter all the information each time. And despite other experiences in this thread having a very prompt turnaround time, it’s been crickets for me and the posted entries are still incorrect.
