Called again the other day. This time, a different agent said that "oh, this one has not been extended, let me see what I can do. We can only extend a certificate once." After a few minutes, it was extended / reissued. She didn't even give me the "we are only doing to because of your status" speech. However, she emphasized, as they have always, that the extension is a one time deal for the certificate.

So, definitely a "if it does not work, try and try again" thing.
