It's cash only, £200 each, doesn't matter how you paid for your original UC ticket (miles, cash, both), exactly 14 days before departure. Just go to the seat map two weeks out and you'll be able to select the seats and pay.
When you close the doors to the suites and lower the middle divider, it really does feel like a huge space. We ended up booking it for the return journey too.
