Not really. Providing it is all on the same booking, makes no difference. We recently did MAN-LHR- overnight at our own expense at hotel not part of booking-LHR-KEF as part of the double tier BA holidays booking.
Had 5 day car hire in Iceland, so got double tier points with no issues.
