There has been a lot of uncomfortable news in the past few weeks from UAL and other domestic carriers, often praised for their safety and great deal of experience in the front of the plane. It is always coming from a (reliable) third party, as the airlines/FAA/NTSB seem to believe it's all not that concerning, which conveniently also keeps these incidents hush-hush. That begs the question of whether more incidents are happening or just getting reported more.
Statistically, the two in the front of the plane are now the ones we should be most worried about. If confirmed and verified, both this incident and the previous UAL incident in the same region are likely to be attributed to pilot error. Luckily we can rely a great deal on automation to keep the flight deck in check, but automation can't fix every mistake that is made. Given the record-breaking airfare levels and profits, UAL has zero excuses not to review any lapses in flight crew safety performance.
