Did they change this? I thought for mileage upgrades if the international upgrade did not clear but the domestic leg cleared then they refunded all the miles as well as the dollars, providing all one upgrade request. When did this change?

I must admit i have not used miles for an international upgrade since before COVID as I have had sufficient PlusPoints and the rule you describe does apply for the use of PlusPoints.
