It's "instant" if the space is available at booking, per the rules.
"Instant upgrades are confirmable at time of ticketing, if space is available. If a seat is not available at time of ticketing, these upgrades will be processed automatically starting at the five-day window. Instant upgrades are subject to capacity controls."
Beginning at the 5-day window, you're in the upgrade priority scrum. You'll want to see if PN>0 if you want the instant upgrade when booking (which is not as rare as PZ, but still rare hub-to-hub). If you see space available after booking (and prior to 5-day CPU window), you can grab the upgrade online, but you have to constantly watch for it. UA will not do the upgrade automatically until the CPUs start running, at which time waitlist rankings doom a lot of YBM-up hopefuls..
The $399 is to basically bypass the upgrade waitlist which, hub-to-hub, sometimes is the only option if one wants to sit upfront.
