1.simply add one US demostic route like NYC to SAN with no extra miles

2.add one more asian route like NRT to SIN or HKG with adding 5000 extra miles

3.combine both option 1&2 with adding 5000 extra miles

I've heard about option 1 can probably be achieved by calling the agent.
What about the other two?

and one last question...I learned about jal is flying two 788 configurations on SAN-NRT.
Are they sticking on Sky suite now or sometimes there will be neo seat?

1) Yes, with the exception of Hawaii.
2) Mostly yes.
3) Mostly yes

I say mostly for 2 and 3 because JAL sometimes implements married segments for its Asian routes. So long as you are not in conflict with a potential JAL's married segment limitation on the Asia segment the answers are yes for 2 and 3. If you run in to married segment restrictions on the Asia segment, one way to bypass married segment limitations is to create a stopover in Japan, because then you have separate trips that are not subject to connecting marriage restrictions between the two trips. Potentially also relevant to remember is that you can only have one stopover, either at JAL's US departure point or in Japan, but not both.
