The flight has to have been in the last 3 weeks, and completing the questionnaire and including sufficient contact info also enters you in a drawing for 2 economy tickets worldwide (1 prize/year) or 5000 Skymiles (awarded monthly). Interestingly, Delta values the ticket at $800, depending on itinerary, and values the 5000 Skymiles at $60 (1.2 cents per).
