How does an incorrect seat number appear on boarding pass?

I’ve got a flight on BA261 later today. It’s an Avios booking, originally in World Traveler with a seat in row 20 reserved. I checked in on the app last night and got a digital boarding pass. It stated World Traveler Plus but with seat 3B.

I’ve never heard of a double upgrade before so I’m sure the seat number is incorrect, and looking on the website it displays a seat in row 11.

I’m wondering if anyone knows what’s happened ‘behind the scenes’ to list the seat in row 3? I’ll head straight for row 11 onboard to avoid the ‘walk of shame’ but is this a frequent occurrence? Surely must be frustrating for the crew to have to sort out if it is.
