No, any economy class paid ticket is fine.
Yes, as per redemption chart but be aware that the agents/robot can count it separately, 15k+45k, so if you dont want to send the feedback and wait, just accept IST-IAH segment upgrade for 45k miles.
Search I class availability at their website or through Expertflyer(accuracy not guaranteed). Purchase tickets through Chase travel to be on the safe side.
You can upgrade all three tickets using your miles from your account.
