hmm why would the difference be refunded? DEN-KOA, while not CPU eligible, is classified as United First. Per rules:
...a MileagePlus Upgrade Award will be considered used if any segment of your travel is upgraded. However, if your travel includes requested upgrades to United Polaris® business class, and none of the international premium segments are upgraded, then your MileagePlus Upgrade Award will not be considered used.
Since DEN-KOA is not Polaris, if MSP>DEN is upgraded, MUA is considered used and no refund will be given. You may be able to only waitlist for DEN-KOA using MUA.
